Here, S₀ is the baseline survival value. Mean is the group mean. Individual Sum comes from the user's risk data.
The Individual Sum uses natural log values. The main terms are:
Individual Sum = age term + cholesterol terms + HDL terms + blood pressure terms + smoking terms + diabetes term
The exact terms change by sex and race group.
Your calculator has four PCE groups. They are White or Other Male, White or Other Female, Black Male, and Black Female.

So, this example gives an estimated 10-year ASCVD risk of about 5.74%.
